WRAP

Description
WRAP is a family-driven, multidisciplinary, team-based approach to help youth at risk of out-of-county placement in higher levels of care.  There is a focus on development of natural support to increase long-term sustainability.  The program requires commitment from family and youth and a willingness to work collaboratively to build a plan of action to meet their needs.

Youth have to be enrolled in CHD mental health services with a minimum of an assessment and treatment plan.  Everything else is driven by the family and youth preference in meeting their needs.

Demographics
Youth ages 0-18 who are at risk of out-of-county placement in a higher level of care.  EOCCO clients.

Location of Service
WRAP can provide services in any setting preferred by the family and youth.
