Wraparound Family Partner/Child Case Manager

Center for Human Development Inc, is seeking a part-time (20 hours per week) benefited Home Visiting Program Specialist.  This position coordinates Home Visiting activities and planning within the team structure to ensure a smoothly functioning program with good internal and external customer relations. Other duties include working on community based programs with community partners, state officials and other programs within the CHD.

High School diploma or equivalent.

One-year related experience.

Excellent communication skills and ability to work well with others from a variety of disciplines.

Demonstrate critical thinking, respond to changing needs, provide leadership to create key values and shared vision, apply effective problem solving skills, and balance multiple priorities.

Must possess a valid Oregon Driver’s license and pass criminal history check and drug screening.

Essential Functions
Serve as a central point for referrals.

Ensure all program guidelines and administrative rules are being followed.

Serves as resource in team environment.

Participate in meetings such as ACCESS, weekly and monthly team meetings, chart reviews, etc.

On-going coordination with nursing staff and family advocates to assist in getting resources needed in completing their jobs (scales, handouts, etc.)

Implement new and revised office and clinical procedures as needed; maintains up-to-date written procedures.

Serve as the fiscal representative for the team.

Meet with state consultants when needed to verify TCM concerns/questions, program expectations, or other questions around services we provide.

On-going outreach and communication with local partners and professionals.
