Description
Center for Human Development, Inc. is seeking a full-time Home Visiting Nurse to provide case management/home visit services for pregnant and post-partum women and newborn infants, children ages 0 to 4 and, children ages 0 to 21 who have special health care needs and fall into the category of established risk, and their families.

The Requirements
Licensed by the State of Oregon as a registered nurse.

One year of experience in community health, public health, or maternal child health nursing preferred.

Thorough knowledge of nursing practices, procedures, public health nursing skills, interviewing and counseling techniques.

Ability to communicate and coordinate services with family advocates.

Communicate effectively verbally and in writing.

Must possess a valid driver’s license and pass all criminal history check requirements and drug screenings.

The Essential Functions
Performs public health nursing services at homes, and other settings to clients in such areas as parent/child health, developmental screenings, family planning, nutrition, communicable and chronic diseases.

Performs client assessment; a documented, systematic and ongoing collection of data to determine current status and identify needs in physical, environmental, psychosocial, developmental, educational, behavioral, emotional and mobility areas.

Coordinate case management services provided to clients with Family Advocates.

Maintains contact with client’s primary care provider and other professionals on behalf of the client.

Advocates for client as necessary.

Provides case management services; an ongoing process to assist client in obtaining access to and effective utilization of necessary health, social, economic, nutritional and other services.

Establishes and maintains client medical chart and other necessary records.

Develops and maintains a client service plan; a written, systematic plan of care listing goals and actions required to meet needs of client.

Initiates follow-up care and makes referrals to physicians, other community agencies or health resources as necessary.

Liaison with Child Development and Rehabilitation Center (CDRC) and OHSU.

Attend case staffing meetings with nurses and family advocate.

Must maintain positive and professional working relationships with co-workers, clients and other agencies.
