Developmental Disabilities Service Coordinator

Center for Human Development, Inc. is seeking a full-time Developmental Disabilities Service Coordinator to deliver case management services to individuals with developmental disabilities and assist the program manager in monitoring the quality of services delivered within the county. Includes determining eligibility and assisting in applying for and obtaining services and benefits, and providing support and information to individuals and family members.

The Requirements
Bachelor's degree and two years of work experience in human services; or five years of equivalent training and work experience.

Knowledge of the public service system for developmental disability services in Oregon.

Ability to communicate effectively both verbally and in writing.

Ability to work effectively with and establish rapport with individuals with a developmental disability.

Ability to prepare clear and concise reports and maintain accurate case notes, treatment plans and client files.

Must possess a valid driver’s license and pass all criminal history check requirements and drug screenings.

Willingness to work flexible hours.
