Behavioral Health Prescription Refills
We ask that you contact your pharmacy first for refill needs. If you need additional support please refer to the contacts listed below to assure timely response for refill requests.

Dr. Azita Safai - please call 541-962-8838 Monday-Friday between 9:00AM and 4:00PM a minimum of 5 days before refill is needed.

Lynn Willis, Ginny Elder or Dale McQueeney please call 541-962-8850 Monday-Friday between 9:00AM and 4:00PM a minimum of 3 days before refill is needed.

We will continue to take the time to carefully review your medications at your visit to ensure a safe and high-quality process to ensure a timely renewal of your prescriptions. We also ask that before you attend your regular appointments, that you please look over your medications, to determine if you need any refills.
