CHD offers hope and healing and new beginnings. Services are available for children, adolescents, adults, and their families. We are here to serve you. With walk-in assessment (Access) times, no appointment is necessary!
Mental Health and Addiction Walk-in Assessment (Access) Hours:
Thursdays 8:00am-4:00pm.
All walk-in assessment appointments are on a first come, first served basis.

Billing and Insurance
Oregon Health Plan (OHP) members are entitled to an assessment through our open access process.
Medicare, private insurance or self-pay individuals who are seeking routine (non-crisis) behavioral health services: Please call 541-962-8800 and ask for a phone screening.
CHD is primarily a contracted provider for the Oregon Health Plan members and the Eastern Oregon Coordinated Care Organization. However, depending on service criteria, we are able to bill most other insurance plans. Please check your policy for specific information on your coverage.
CHD may provide a mental health screening to all individuals. Appropriate treatment referrals will then be made based on program and clinical guidelines, family size, household income, and criteria determined by funding sources. For more information, please contact the front desk.
