Description
Center for Human Development, Inc. is seeking a full-time Youth Wraparound Care Coordinator to plan, organize, and direct the youth wraparound program. Responsible for promoting community awareness of program and facilitating client services and coordination of care.

The Requirements
Bachelor's degree in a behavioral sciences field required.

Understanding of mental health assessment, treatment and service terminology and ability to apply the concepts, provide psychosocial skills development, and implement interventions. 

Familiarity with community and regional resources and systems of care. 

Excellent communication skills including public speaking, interviewing and counseling skills.

Organizational skills.

Knowledge of state and local systems and knowledge of youth care intensive treatment methods and practice.

Flexible schedule to include evenings and ability to travel for trainings.

Must possess a valid driver’s license and pass all criminal history check requirements and drug screenings.

General Description of Duties
Engage families in intake and orientation process. Provide coordination of care and facilitate out of home placement as appropriate as a member of a youth and family driven team. Establish partnerships with local community members, schools, physical health, alcohol and drug treatment providers, and any other organization or individual necessary to effectively coordinate the team based treatment process. Partner with local business and agency administrators to develop resources for delivering community based services. Promote community awareness of issue specific to youth risk factors.

Facilitate YFT (Youth and Family Team) meetings. Develop Proactive Safety Plans and implement service coordination plans. Identify barriers to family success and develop strategies to overcome them. Collect and submit data per program guidelines.
